I agree with you in theory. The problem I haven't been able to figure out..
and I've actually given this a good amount of thought...is that a lot of medical procedures have unknowns associated with it. If you are under the knife for a heart procedure, and all hell breaks loose, it's not like they can wake you and ask you if it's ok if they proceed. They are going to do everything in their power to fix the problem and save your life.
Child birth is similar given the amount of emergency (and probably unnecessary) c-sections being done. You can't always know the services upfront.
I agree with everything else though. We have become way too focused on providing people with insurance when the problem is actually that the insurance has given hospitals (and to a much lesser extent) the ability to inflate prices at absurd levels.
I've been in multiple fights with hospitals to see itemized bills and the whole thing is appalling.
